Picture Upload REQUIRED:

A picture is required to register. (players and admins)

Picture must be a HEADSHOT only. Full body shots will be rejected.

Picture will be looking forward, NO HAT, no face paint, no dark shadows. No trophy or medal.

DO NOT Upload Birth Certificates:

Those new to UYSA will be required to provide a birth certificate (BC) to the registrar. 

Any government issued document that includes full legal name and birth date are acceptable.

Any language acceptable thanks to Google Translate.

Create picture of BC. (PDF, JPEG, etc.) File size must be between 250 KB and 5 MB.

Email Birth Certificate to registrar@utahsurfsoccer.com.

 
Deadline:

Registration deadline is June 21, 2025, 11:59 PM.

You may still register after this date but a $20 late fee will be added.
